Tilt-shift photography comes with the use of tilt for selective focus and camera movements, for simulating a scene in miniature. The lens plane is totated relative to the image plane (tilt), while lens are moved parallel to the image plane (shift). Tilt controls the plane focus orientation, shift adjusts the subject position in the photo, in order to avoid the parallel lines to converge (for instance when taking photos of tall buildings).
Some photographers give a Photoshop touch to obtain the same effect, but either way, the result is really amazing (not to be read that I agree with using Photoshop everytime everywhere). Here are some interesting examples of real and fake tilt-shift photography, for your inspiration.
